There are cute personalized options for tennis, basketball, football, soccer, volleyball (a second volleyball option here), dance, ballet, gymnastics, cheerleading (a second cheer option here), or only a personalized name. They are all reasonably priced so that you could also include a gift card to a local sports store with this gift.

For younger kids there are smaller 10oz bottles available from Etsy shop, Shop Around The Corner 3, like these clear, BPA-free, straw bottles that can have a name and sport icon added. What is nice about these bottles is that there is an option to use a tinted color for the inside bottle like with these gymnastics bottles with a pink inner layer. Other activities available include ballet, cheer, basketball, swimming, karate, hockey or a name without an icon.

I ultimately ended up ordering from Zazzle because they had a basketball version that is BPA free and made of stainless steel, but it is more costly than the Etsy options. When I ordered this bottle, the default has the 18 oz bottle selected, but there is also a 24 oz option available for additional cost. I didn't think much of the size at the time, but after receiving it I learned that the 18 oz size is smaller than what I'm used to seeing for these bottles. The 18 oz size is good for a kid, but for a teenager, I would probably opt for the 24 oz size.
